Case management helps older persons, adults with disabilities or their caregivers coordinate all the services that they may need in order live in their homes or care facilities. The case manager may assess the client’s needs, develop a care plan, authorize and arrange for services, coordinate the provision of services among providers, monitor care, and help with discharge from care facilities.Visit Website for list of service providers
